How to Enter:

1. Have a professional 5X7 color photo taken of your dog dressed stylishly in a "party setting," for example, but  not   limited to, a birthday party, barbecue, graduation, pool party, puppy shower, or other favorite social gathering.

2.  Download and print out the following forms:  (Select desired format, right click on it, and select "save target as")

• Official Rules -                

• Photographer's Release and Assignment of Copyright -            

Note: You may obtain these forms by mailing a stamped, addressed envelope to: America's Top Dog Model Contest, P.O. Box 273525, Boca Raton, FL 33427-3525.

3.  Fill out all forms.  The "Photographer's Release and Assignment of Copyright" form should be signed by your photographer.   The "Official Rules" is to be signed by the Entrant (Contestant).

4.  Photo and all forms must be sent via U.S. Mail to: America's Top Dog Model Contest, P.O. Box 273525, Boca Raton, FL 33427-3525.
