There are two ways to enter the contest:

1. Click “Official Rules” and read and understand all contest rules and requirements.

2. Right-click and save your preferred file format (.DOC or .PDF) of the “Dog Profile and Rules” document.

3. Print out document.

4. Complete Dog Profile form and sign and complete the Rules page.

5. Have a professional-quality photo taken of your dog.

6. Right-click and save your preferred file format (.DOC or .PDF) of the “Photographer’s Release and Assignment of Copyright” document.

7. Obtain a completed and signed document from your photographer.

8. Mail “Dog Profile”, “Rules”, “Photographer’s Release”, and photo to:

America’s Top Dog Model ® Contest P.O. Box 273525

Boca Raton, FL  33427-3525.

All mail-in entries must be postmarked on or before April 30, 2013 in order to be eligible.

3. Fill in all required information

4. Use specified box to attach a professional-quality photo of your dog.
